Care Coordinator (Mental Health) Part-time
About this role
Northern District Community Health is looking to employ a Care Coordinator to support a Nurse Practitioner with a mental health specialty. The role is part of an Innovative Models of Care program being conducted across the Buloke, Loddon and Gannawarra Shires involving a partnership between health services, general practitioners and other nurse practitioners.
This position will be based within the East Wimmera Health Services catchment area and will be supported by a Northern District Community Health team members, East Wimmera Health Services and a network of other care coordinators.
The successful applicant will have qualifications in mental health, or alcohol and other drugs, or peer work, or welfare or currently working towards a relevant qualification, as well as lived experience of mental issues and recovery (as direct experience or as a carer).

About you:
Essential
- Qualifications (minimum Certificate IV) in Mental Health or Alcohol and Other Drugs or Peer Work or Welfare or currently working towards a relevant qualification.
- Lived experience of mental health issues and recovery (as direct experience or as a carer).
- Genuine care for people and personal qualities that align to NDCH’s values.
- Current driver’s licence.
- Current Police Check, NDIS Screening Check and Working with Children Check or willingness to obtain one.
Desirable
- Understanding of and/or experience with community and consumer engagement.
- Interest and/or experience in working with Nurse Practitioners.
- Other skills, competencies and/or qualifications relevant to primary health care scope of practice and setting.
