Chronic Disease Management Nurse
Part Time Position - 0.4EFT to June 2021
Northern District Community Health (NDCH) is seeking to employ a Chronic Disease Management Registered Nurse, 0.4 EFT until 30 June 2021.
The role will be based in Kerang but will service the other surrounding towns. The Chronic Disease Management Nurse will work with people who have chronic diseases to deliver connected, safe, effective and person-centred care in accordance with care plans developed in conjunctions with the client, their General Practitioner and their healthcare team. The role will be supported with short on line courses focusing on skill development to support people to self-manage their conditions.
The successful applicant will work within the broader NDCH multi-disciplinary team that includes other Nurses, Allied Health Professionals, Medical Clinic and Health Promotion team members.
NDCH is an integrated Community Health Service and Medical Clinic that delivers services into Gannawarra, Buloke and Loddon Shires. NDCH prides itself on being an inclusive and safe place for both clients and employees, with a strong and defined workplace culture that supports team members.
A drivers licence is essential.
