NDIS
National Disability Insurance Scheme

NDCH NDIS Request for Service/Referral Form

What is the NDIS?

The NDIS is designed to give Australians living with disability the reasonable and necessary supports they need in every day life.

Everybody's needs and goals are different, but the overall aim of the NDIS is to help you:
  • make your own choices
  • have control
  • be more independent
  • be involved in your community
  • in getting a job
  • improve your well-being

NDCH is a registered NDIS provider!

An NDIS Plan allows a person to receive core, day-today living support for a range of activities so they can start or continue to participate in the community.

Participants may also be entitled to capital supports, which provide access to resources or equipment such as wheelchairs, assistive technology and home or car modifications. There will also be an option for capacity building supports that will help to develop life skills, educational opportunities or help to find employment.
What supports does NDCH provide under the NDIS?
We are registered to provide the following NDIS supports:
  • Occupational Therapy
  • Community Nursing Care
  • Podiatry
  • Dietitian
  • Speech Pathology
  • Support Coordination
  • Group Programs
You can choose supports from us.  You can choose supports from another service.  Or you can do a bit of both.

Are you eligible?
To be eligible for the NDIS, clients need to:
  • live in an area where the NDIS is available;
  • be under the age of 65; and
  • have a disability that affects their ability to perform every day activities.
