The role of a Community Health Nurse (CHN) is to deliver quality professional service to help people in the community to achieve their health and wellbeing goals.
Our Community Health Nurses are a multi skilled team of professionals. Their tasks include:
Diabeties Resource Nursing
Our CHN’s provide support to our Diabetes Educator to assist with routine Diabetes reviews and provision of support for clients with Endocrinology Telehealth appointments. They also assist clients with education and improvement in managing their diabetes and meeting their individual wellbeing goals.
Cardiac Rehabilitation Nursing
Our Cardiac Rehabilitation Program offers education and exercise for clients who have cardiovascular disease or have had a cardiac event.
Continence Support Nursing
Incontinence is a term that describes any accidental or involuntary loss of urine from the bladder (urinary incontinence) or bowel motion, faeces or wind from the bowel (faecal or bowel incontinence). Our CHN’s can assist with the treatment and management of incontinence and in many cases assist towards a cure.
Wound Management Nursing
Our skilled community nursing team provide wound management services both in the home and within our clinics. The service provides both wound assessment and treatment for people who have acute or chronic wounds, and / or who have an increased risk of complications due to underlying health conditions.
Respiratory Management Program
Our Respiratory Management Program assists clients with problems or issues associated with breathing and lung health.
