Diabetes Connect Program - Diabetes Educator

Northern District Community Health is employing a Diabetes Educator 2 days a fortnight until 30/06/2025. The role will work in the Diabetes Connect Program with three other healthcare professions that include a Program Coordinator, Dietitian and a Psychological Support Worker. The successful applicant will work with Medical Clinics to increase access for their patients who live with Type 2 Diabetes to be able to self-manage their condition through virtual monitoring and social prescribing. Professional development opportunities to support this role are also available. This role will be working within the Gannawarra Shire and Swan Hill Rural City Council.
This role is supported by a diverse Allied Health Team including two well-experienced Diabetes Educators as well as the broader NDCH team.
