Diabetes Connect Program - Psychosocial Support Worker

Northern District Community Health is employing a Diabetes Connect Program Psychosocial Support Worker 2 days a fortnight until 30 June 2025. The role will work in with three other Diabetes Connect Project Workers that include a Dietitian, Diabetes Educator and a Psychological Support Worker. This role will be working within the Gannawarra Shire and Swan Hill Rural City Council. The successful applicant will work with Medical Clinics to increase access for their patients who live with Type 2 Diabetes to be able to self-manage their condition through virtual monitoring and social prescribing. Professional development opportunities to support this role are also available.
This role is supported by a diverse Allied Health Team including two well-experienced Diabetes Educators as well as the broader NDCH team.
A current Victorian driver’s license is mandatory.
