Diabetes Educator
Part Time Position
Northern District Community Health (NDCH) is seeking to employ a part time Diabetes Educator for 8 hours per week. A skilled and motivated person is required to work with individuals and their families particularly people with Type 2 diabetes.  The successful applicant will be required to work within a multi-disciplinary team that includes another Credentialed Diabetes Educator, General Practitioners and Chronic Disease Management Nurses.

The position will be cover NDCH catchment area of Gannawarra and Northern Loddon.

Applicants must display a high degree of nursing skill focusing on people with diabetes.  Also a strong commitment to early intervention and the ability to engage people and their families around self-management of their chronic disease state. Prior knowledge of Health Promotion is desirable but support from management and team members will be available to applicants who undertake this role.


NDCH is an integrated Community Health Service and Medical Clinic that delivers services into Gannawarra, Buloke and Loddon Shires. NDCH prides itself on being an inclusive and safe place for both clients and employees, with a strong and defined workplace culture that supports team members. 

A drivers licence is essential.
