Healthy Hearts & Lungs Program Facilitator

Northern District Community Health is looking to employ an Exercise Physiologist or Physiotherapist 2 days a week to facilitate the Cardiac & Pulmonary Rehabilitation Program.
The successful applicant will be leading an established program that also includes a Program Support Worker. The Cardiac & Pulmonary Program Facilitator will be working within a team of other experienced Allied Health Team as well as the broader NDCH team.
This program aims to connect with community members who would benefit from a short face to face course to increase their knowledge and skills to improve their cardiac, pulmonary or diabetes condition.
The successful applicant will need to have completed the necessary Cardiac and Pulmonary Rehabilitation training or have completed it within six months of commencing employment to deliver this program. Professional development support will be available to complete this training if needed.
A commitment to teamwork, demonstrated skills in evaluation and highly effective communication skills are a must in this role.
