Northern District Community Health is seeking a Manager Home Care to lead the expansion of our community care team. The team operates across both Swan Hill and Gannawarra and offers the right applicant a great opportunity to support people in rural communities.
The successful applicant will have knowledge and a successful track record of working in aged care, NDIS and other care@home models and will have a deep understanding of the delivery of the highest quality care for vulnerable people. The leadership experience that you have will ensure that the program is well managed and meets quality standards and performance outcomes.
This person will be part of a diverse Community Health and Community Home Care Team and will be well supported by the broader NDCH team that includes Nursing, Community Support, Health Promotion, Allied Health and Community Home Care. A commitment to interdisciplinary teamwork and highly effective communication skills are a must in this role.
A generous salary package is available for this role including financial assistance with professional development costs, including covering some attendance costs and the use of a fleet car.
A current Victorian driver’s license is mandatory.
