Sustainable Rural Healthcare Hubs Program Facilitator
80 hours with monthly rostered days off
Northern District Community Health is seeking a Sustainable Rural Healthcare Hubs Program Facilitator to project manage the planning, implementation, and evaluation of a trial model of care for Buloke, Loddon, and Gannawarra Shires. The expected outcome of this pilot project is to research the acceptability and sustainability of Nurse Practitioners working into existing Medical Clinics to increase access of care for smaller rural communities in the above mentioned shires.
The successful applicant will be expected to have well developed program management skills as well as extensive knowledge of both the clinical and corporate workings of Medical Clinics. A substantial part of this role will be in research as this is a pilot program so researching skills will be essential.
This person will be part of a diverse Nursing Team and will be well supported by the broader NDCH team that includes Nursing, Community Support, Health Promotion, Allied Health and Community Care. A commitment to interdisciplinary teamwork and highly effective communication skills are a must in this role.
A generous salary package is available for this role including flexible client workload, financial assistance with professional development costs,- including covering some attendance costs- and the use of a fleet car.
A current Victorian driver’s license is mandatory.
