Sustainable Rural Healthcare Hubs Project Coordinator
Northern District Community Health is seeking a Sustainable Rural Healthcare Hubs Project Coordinator to support the planning, implementation, and evaluation of an innovative trial model of care for Buloke, Loddon, and Gannawarra Shires called “BLG Sustainable Rural Healthcare Hubs”.
The Project Coordinator will work with the project management team including Murray PHN and Monash University to implement a new multi-disciplinary team-based model of care at six sites across the region, which aims to improve community access to primary health care for health promotion, illness and injury prevention, and chronic disease management.
The project is being delivered by the Integrated Health Network Alliance, a cross-sector partnership of health service executives committed to building health system sustainability through collaboration and integrated service planning, delivery, and evaluation.
The project is funded through the Department of Health and Aged Care, and Office of the National Rural Health Commissioner, Innovative Model of Care grant program, which aims to trial new ways of providing primary care in rural and remote communities. This provides a unique and exciting opportunity for the successful applicant to be a part of a nationally recognised trial model of care that will have impactful findings nationally at a policy level and locally for the health of small rural communities.
The successful applicant will be expected to have project management skills as well as knowledge and interest in primary health care models and settings, and research/evaluation or quality improvement.
The role will be well supported by the broader NDCH team that includes Nursing, Community Support, Health Promotion, Allied Health and Community Care. A commitment to teamwork and highly effective communication skills are a must in this role.
A generous salary package is available for this role and flexible work arrangements, financial assistance with professional development costs, - including covering some attendance costs- and the use of a fleet car.
A current Victorian driver’s license is mandatory.
